SVM “In Memoriam” Program

Are you a veterinarian who would like clients to know you share their loss?

Veterinarians who wish to recognize the loss of clients’ pets find the In Memoriam program valuable and easy to use. Once a clinic is enrolled in the program,

they simply send us the necessary information with their donation, and their clients will receive a personalized memorial card in the mail.

Recipients of these cards often write to us or their veterinary to express how much this card has meant to them.

“Thank you for your card and your caring thoughts about our dog, “Susie.” She will be missed. The ….. Animal Hospital has been treating our pets for over

20 years. I hope all of you students in the veterinary program achieve your goals and become outstanding people like the ones at …... Animal Hospital.”

Dear Dr. ….., My husband and I (received) a poignant card from the Purdue School of Veterinary Medicine, telling us that you made a donation in our dog’s name. We cannot tell you how much your donation means to us. You understand that having uch a wonderful creature in our lives for16 years, and then being without them is a part of the life cycle; but is also one of those devastating events in one’s life. You are also one of God’s special creatures. Thank you.”

Veterinary Clinics may enroll in the “In Memoriam” program by printing and completing the enrollment form and mailing it to:

Once enrolled, simply send your contribution check, made payable to Purdue Foundation, to the address above along with a list containing the name and species of the pets, and the name and address of the pet owners. We provide a form to make it easy to submit your list of clients. A copy of the form is available in PDF format and Word format.

You may also submit your list electronically. Complete this Excel form and email it to vetalumni@purdue.edu. Payment can be made with a credit card or check. If you choose to mail a check, your list will be processed upon receipt of payment. Instructions are on the form.

Veterinarians choose the amount they wish to give. Because of the program costs, we recommend a minimum gift of $10 per pet. You will receive an official University receipt, and each client on your list will receive a beautiful, personalized card notifying them of your monetary gift made in memory of their pet.
