Guidelines and Forms

As of January 2010, there are new Pet Food Proposal Guidelines in place. Please click below for access to the guidelines and forms in either Microsoft Word or PDF format. These forms are necessary in order to be eligible for the Pet Food Funds.

Pet Food Money

If you are going on a trip related to veterinary medicine (study abroad, veterinary conference, etc.) then these are the forms you need to fill out:

• Individual Education Experience Form
• Travel Planning Form
  Turn in receipts for your travel (one separate from others traveling with you)
  Checklist if you need help.

Educational or Social Event:

If you are an officer of a club planning an event and want to apply for money to help make your event amazing, you need to fill out the following forms:

• Event Pet Food Proposal
• Event Planning Form
• 20 Signatures from Students in the vet school
