Information for DVM Class of 2017 Applicants

We are very excited that you have chosen to apply to the DVM program of the Purdue University College of Veterinary Medicine.  This page will provide you with updated information for this application cycle. Specifically on this page is a link to the "Course Verification Form" which is referred to on VMCAS as our "supplemental application". There are no additional fees involved.

Special Instructions and Form Submission Options

• The Course Verification Form allows you to indicate what course(s) meet the Purdue admission prerequisite requirements. It is extremely important that you complete this form in its entirety as any deficits may result in the delay in processing of your application.
• The submission deadline is October 2, 2012
• Submission options:
  
  • If your computer is currently running a standard mail client (i.e. Microsoft Outlook), then you may use the "submit" button at the top of the form. The form will be emailed to our office automatically.
  • If you are using a web-based email client application (i.e. Gmail, Hotmail, etc.) you will need to save the form and then submit electronically as an attachment to vetadmissions@purdue.edu
  • Should these submission options not work for your computer, you may print the form and fax it to the Student Services Center at 765-496-2891. The preferred means to receive this form is the electronic submission.
