School of Veterinary Medicine Teaching Hospital Clients Thank you for allowing the Purdue University Veterinary Teaching Hospital the privilege of treating your animal. Every animal is important to us so please know they will be given excellent care. Our Referral Policy We welcome referral appointments! Your animal's health care needs can best be met by establishing a professional relationship with a veterinarian near your home. The primary care veterinarian and the veterinarians at the Veterinary Teaching Hospital can then work as a team with you and your animals to provide continuity in patient care. To facilitate this relationship, we ask that you bring with you copies of pertinent information regarding your animal's condition, including test results, films and treatments. It is not necessary for the primary care veterinarian to call to make the appointment. Clients may do so themselves. However, it is very important to know to which specialty your primary care veterinarian is referring your animal. When you call to make the appointment, you will be asked the name of the primary care veterinarian who is referring you, what specialty your veterinarian wants your animal to see (example: cardiology), and the specific problem for which the animal is being referred. You will also be reminded to bring all the referral information from your veterinarian with you when you come to the Veterinary Teaching Hospital for the appointment. Policy For Communication With Referring Veterinarians The clinicians of the Veterinary Teaching Hospital communicate with the referring veterinarians by phone and in writing. A copy of the dismissal instructions given to you at the time of your animal’s discharge from the hospital is sent to your veterinarian the next business day. Appointments We offer general medicine services (annual checkups and vaccinations), specialty services, and welcome referrals from other veterinarians. To facilitate your visit, we request that all our clients, including those referred by a veterinarian, schedule an appointment prior to arriving. If for some reason, you are unable to make the scheduled time please notify us. Please bring with you the referral instructions and copies of the lab work, radiographs (X-ray) and any other information from your veterinarian. Cancellations If for some reason you are not able to keep your scheduled appointment, please call the appropriate reception office at least 24 hours in advance to cancel. This allows us to see another patient during that time period. Thank you.
Refund Policy Refunds for credit balances greater than $5.00 will be made on a case by case basis. Credits under $5.00 will be applied toward future visits. Please advise us if you would like for your credit to be donated to the Veterinary Teaching Hospital. Parking Parking is provided east of the Small Animal Hospital entrance and west of the Large Animal Hospital entrance. Park in front of the signs marked - VTH Client Parking. Please provide your license plate number at check in. Parking is free.
